See calendar for all events. This form is for all events that require registration ahead of time. Please turn in by appropriate date as listed on form, depending on events attending.
Medical Release Form 2012-2013
The medical form is required for all events and retreats. It is good for a year and kept on file, so you only need to fill out one per year. (June – May)
